- Based on documentation and in collaboration with the student, accommodations will be provided with an Individualized Service Plan (ISP) form.
- Each semester, the student is expected to request an ISP; this form will be sent electronically to the student’s course instructors.
- The ISP includes the student’s name, program of study, semester, accommodations, and who they are working with.
- ISPs can be amended or created at any time during the academic year.
- Accommodations are based on a permanent or temporary disability, but due to privacy laws, the student’s disability will not be shared with faculty without direct consent from the student.
