Centre for Healthy Communities

The Centre for Healthy Communities (CFHC) uses social innovation to tackle urgent challenges in health care, social services and community well-being. The CFHC collaborates with community not-for-profits, government agencies, health, and social service partners to strengthen their impact on social determinants of health, through a health equity lens, that aligns with the college’s commitment to the United Nations Sustainable Development Goals (SDGs). The foundational pillars of the CFHC include supporting four SDGs: reduce poverty, increase good health and wellbeing, increase quality education, and reduce inequalities. The CFHC is a “centre without walls”, meaning research projects and activities are integrated in community with partners and service providers to promote ease of access and collaboration. 

Services

The CFHC is equipped to help community partners solve real-world challenges with immediate applications through the support of data-informed decision-making. Types of services and projects supported through the CFHC include: 

A structured process of collecting and analyzing information about internal and external factors that could affect an organization, project, or decision. They help leaders identify risks and opportunities, align resources, and understand the current landscape before planning strategy or making changes. 

A process used to identify and understand gaps between what currently exists and what is needed in a community, organization, or population so that it can be understood what supports, services or changes are required. 

An analysis used to determine whether a proposed project, program, or idea is realistic and likely to succeed before investing resources into it. 

Process of planning how a program will work so it can achieve its goals and address a specific need by offering a blueprint for how it will operate and create change. This service stream is often accompanied by an environmental scan or needs assessment to support design, or a trial and evaluation to measure impact. 

A small-scale test of a program before it is fully implemented to see if it works as intended and to identify any improvements needed. This service stream is often accompanied by a program/service model design and/or evaluation 

A systematic process used to assess how well a program, project, policy, or initiative is working to understand whether goals have been achieved and the impact it has. 

Process of taking research, information, or evidence and actively sharing it so it can be used to make decisions, improve programs, or influence policy. This component is included within all service streams and may involve community engagement elements. Knowledge mobilization involves getting the right knowledge to the right people at the right time so it can be applied in a real-world context.  

The Centre for Healthy Communities was exceptional to work with on our research project. They were professional, ethical, organized and responsive to our needs. Their ability to understand our needs, translate them into a research project and lead our project to produce a well-crafted, academic, white paper report at the end was beyond expectations. The team was personable, supportive and very easy to work with. I highly recommend this centre to everyone working in the health and wellness sector!
