Credential request form

Please use this form to request a new copy of your printed credential (e.g. diploma or certificate). The cost for a new printed credential is $37.45 (tax included). If requesting more than one credential, $37.45 applies to each copy being requested. If you are ordering two credentials, the total cost is $74.90. If you would like your credential mailed to an address outside of Canada, an additional $25 charge applies.

Note: if you have a hold on your account or previous balance owing, payment will go towards any amount owing and your credential request will not be processed.

You can confirm if you have a balance owing through myLoyalist > Banner > Student Services > Accounts Receivable. If you attended classes more than three years ago, you can log in to Banner directly: loyalistbanner.com. Your Banner ID is your student number and your PIN was initially your date of birth in the format YYMMDD. Please have your student number ready when making payment. If you do not know your student number, please call 613-969-1913 ext. 2100.

Payment Methods:

•Your bank (in-person or through online banking): Payee: Loyalist College. Account Number: your Loyalist College 9-digit student number.
•In-person (credit, debit, or cash): Payment Office (Room 2H1), Monday – Friday 10 am – 3 pm

Your name will appear on your credential as it appears on your student record. If you wish to change any of your personal information on file, please log in to Banner using the link above.

All fees must be paid before submitting this form.
